Insurance / Payment

The following payment options are available:

• Insurance: Dr. Hoffman is in-network with most commercial UPMC Health Plan and Highmark / Blue Cross Blue Shield plans.

  • Contact your insurance to clarify your benefits by following these steps.

  • Dr. Hoffman is not in-network with UPMC for Kids, UPMC for You, Highmark Healthy Kids, Highmark Wholecare, or any other CHIP or Medical Assistance (MA) policies.

  • For other insurances, check with your insurance provider regarding out-of-network reimbursement. To use out-of-network benefits, you will pay upfront for the cost of the evaluation. You will then receive a detailed receipt that you can submit to your insurance provider for reimbursement at the rate they have established for out-of-network benefits.

Academic Testing

Academic testing is not covered by insurance. However, in cases where there is a clinical concern (e.g., anxiety, ADHD) as well as an academic concern (e.g., learning disorder), a portion of the evaluation may be covered by insurance, but there will be an additional academic testing fee. Cancellation Policy

Scheduled appointments must be cancelled or rescheduled at least two weeks in advance of the appointment. Late cancellations will result in a cancellation fee, which is not covered by insurance.

Credit card, cash, or check are all accepted for payment.
